What this notice covers
This notice covers the information you choose to send through the enquiry form on this website. Information created during your medical care is held separately as part of your clinical record and is governed by professional confidentiality and the duties set out by the General Medical Council.
What is collected
The enquiry form asks for your name, email address, telephone number and a short message about what you need. You may also tell us the type of enquiry, how you prefer to be contacted, the area or hotel you are in, and whether you are arranging care for someone else. Please do not include detailed medical information in the form.
How it is used
Your details are used only to reply to your enquiry and to arrange an appointment if you go ahead. They are not used for marketing, and they are not sold or shared with third parties for their own purposes.
Where it is stored
Enquiries are stored securely by the website and sent by email to Dr Azhar, who reads and answers them himself. Access is limited to Dr Azhar.
How long it is kept
Enquiries are kept for as long as needed to respond and to keep a record of the correspondence. If you become a patient, relevant information is transferred into your clinical record and kept in line with professional record keeping requirements.
Your rights
Under UK data protection law you can ask for a copy of the information held about you, ask for it to be corrected, or ask for your enquiry to be deleted. To do that, reply to the email you receive after making an enquiry, or send a new enquiry saying what you would like. You can also raise a concern with the Information Commissioner’s Office.
Cookies and analytics
This website uses Google Analytics to understand how pages are used. That service sets cookies and records information such as the pages visited and the general location of the visit. It is not used to identify individual patients.
